Job Summary Job Title: Social Worker – Acute Care. Employment Type: Full time. Shift: Day (Monday–Friday, 8:00am–4:30pm; on‑call 4:30pm–8am one week per month). Location: SaintFrancis Hospital – Wilmington, DE. Position Purpose The Social Worker monitors the patient’s progress throughout hospitalization and documents relevant findings with regard to Social Services and discharge planning in a timely manner. Responsibilities Establish accountability for data‑driven quality care, service and financial outcomes related to patient transitions of care. Set priorities to improve processes and outcomes related to patient care transitions, focusing on appropriateness, efficacy, efficiency, timeliness, accessibility, continuity, safety of the care environment, and respect for the patient’s personal values, preferences, and beliefs. Prioritize and facilitate timely discharge planning for patients with post‑acute care needs to avoid unnecessary delays. Maintain documentation in the medical record and Midas. Interview patients and significant others within one working day of referral to obtain data on personal, social, medical, emotional, cultural, and religious history; delineate problems requiring Social Work intervention and plan appropriate services. Collaborate with multidisciplinary team members regarding discharge planning; arrange necessary care in the home or an alternate setting. Consult, assess, refer, and coordinate interventions in cases of suspected or actual child or partner abuse, neglect, domestic violence, or sexual abuse. Complete appropriate assessments for assigned patients to identify gaps in care, develop goals, and facilitate interventions and resources. Assist patients and families to complete Medicaid applications. Manage the placement process into inpatient facilities and review long‑term placement financial resources; provide education to the patient and family. Develop plans for transfer to SNFs, acute rehabilitation facilities, assisted living, inpatient substance abuse centers, and inpatient psychiatric hospitals. Facilitate patient/family and provider meetings as necessary to develop strategies related to psychosocial and economic issues, particularly those related to care progression and transition. Act as liaison between patients, families, and all providers of care within and external to the organization on social issues. Work collaboratively with the multidisciplinary team to identify patients at risk for rehospitalization and mediate barriers to a successful discharge plan. Assist the multidisciplinary team in assessing psychological and social aspects of patient care needs and collaborate on plan‑of‑care interventions. Prepare and maintain individual patient and departmental records according to departmental, federal, and state regulations; complete statistical information and prepare correspondence as needed. Adhere to discharge planning and organizational policies and standards, as well as standards from external regulatory agencies and accrediting bodies (e.g., JCAHO, Department of Public Health). Minimum Qualifications Master’s degree in Social Work (MSW). At least one year of experience providing social work services in an acute care setting. Strong leadership ability, excellent interpersonal skills, and solid oral and written communication skills.
