We seek a Physical Therapist to provide home‑based care, including assessment, treatment, education, and case management. Key Position Details 5k sign‑on bonus for external talent 0.5FTE (40hours over two weeks) 8‑hour day shifts, occasional weekends Responsibilities Provide Physical Therapy treatment to clients in the home setting. Initiate and conduct client assessments based on diagnosis. Consult with clients on goals and desired outcomes. Educate clients, family members, or others involved in care about the treatment plan, including age‑specific and cultural considerations. Identify goals and interventions for each problem, impairment, or functional limitation. Continuously assess client needs and functional deficits, progressing the plan of care. Initiate appropriate referrals to other disciplines. Case‑manage clients as needed, establishing and providing ongoing oversight of the plan of care. Facilitate communication with interdisciplinary providers and payer sources. Manage caseload with consideration of priorities, scheduling, and urgency of clients within the geographic area. Perform other duties as assigned. Required Qualifications Bachelor’s degree in Physical Therapy from an APTA accredited program. 2–5 years of experience in a rehabilitation or long‑term care setting. Preferred Qualifications Master’s degree in Physical Therapy. 0–2 years of experience in a Home Care or acute care setting. Licenses & Certifications Licensed Physical Therapist – MN State Board of Physical Therapy (required to provide services in MN). BLS Tier 1 – Basic Life Support (required by completion of orientation). Valid driver’s license required upon hire; MN and WI residents must obtain a license within 60 days of employment. Physical Demands Medium work: lifting weight up to 32lbs occasionally; up to 25lbs frequently. Pay Range $42.92 – $59.38 per hour.
