- Assist with developing preliminary and comprehensive assessments of each resident's dietary needs.
- Develop written dietary plans of care identifying nutritional concerns, resident needs, and goals.
- Encourage residents and families to participate in the development and review of care plans.
- Ensure dietary personnel understand and follow each resident's care plan when providing daily dietary services.
- Identify special equipment and utensils required to meet individual resident needs.
- Review and interpret information in residents' medical charts and provide recommendations for appropriate medical nutrition therapy.
- Collaborate with the interdisciplinary care team to support residents' nutritional health and overall well-being.
- Must be a licensed Registered Dietitian in the State of New Jersey .
- Minimum of two years of professional dietitian experience in long-term care, including some administrative responsibility; or
- One year of experience as a registered consulting dietitian serving one or more long-term care facilities.
