Overview Bilingual Care Coordinator for Individuals With Disabilities — Full Time — Pay: $51,700 per year (nonexempt). United Cerebral Palsy of Southern Arizona (UCPSA) is hiring a Bilingual Care Coordinator in our Green Valley office. This individual will help to maintain and grow our membership in underserved areas in Southern AZ. UCPSA has a diverse and dynamic team as well as a unique company culture. Responsibilities Care Coordinators coordinate in-home and community-based services for the elderly and people living with all types of disabilities. Working Mon–Fri 8a–5p, with occasional schedule variations to accommodate client needs, including direct care shifts, evenings, or weekends. Performing on-call duties at least once every 8 weeks (two consecutive weeks) including after-hours calls and, if needed, direct care shifts. Maintain and grow a caseload in Southern Arizona. Schedule shifts for clients and direct care staff. Orient and train direct care staff with clients. Monitor direct care staff performance. Hold staff meetings as needed and attend weekly program department meetings. Conduct intake of new clients. Meet with clients and families minimally every quarter. Attend meetings with other agencies involved in a client's care. Ensure that all paperwork in client files is updated regularly and document significant conversations and/or correspondences with staff and clients. Maintain client and agency confidentiality at all times. Qualifications 18 years of age or older. Ability to obtain a current CPR/FA certification. Ability to obtain fingerprint clearance. Ability to clear a DES background check. Ability to obtain a current TB clearance card. Ability to work in a team environment and handle difficult situations with clients/families. Ability to work a flexible schedule and to learn and operate UCPSA program software. Ability to work independently and lift and transfer clients. Own or have regular access to a motor vehicle; ability to travel throughout Green Valley, Sahuarita, and surrounding areas. Proof of current registration and insurance must be kept on file with UCPSA. At least one year of direct care experience, or a degree in a related field, and/or case management experience. Bilingual in English and Spanish required.
