We are seeking an experienced Family Medicine, Internal Medicine, or General Practice Physician to join our team. Why Join Our Team? You will care for patients with complex medical needs while working collaboratively with our clinical team to coordinate care and support the overall health and well-being of each patient. Your responsibilities will include: Evaluating, diagnosing, and treating assigned patients Developing individualized treatment plans Monitoring patient conditions and adjusting treatment as needed Coordinating care with specialists and other healthcare professionals Educating patients and families regarding health maintenance and disease prevention Facilitating referrals and transitions of careMaintaining accurate and timely medical records Communicating effectively with patients, families, facility staff, and members of the care team Building strong relationships with patients and the facilities we serve Our goal is to provide excellent patient care while helping reduce avoidable hospitalizations and readmissions. What We're Looking For Board-certified or board-eligible physician in Family Medicine, Internal Medicine, or General Practice Active, unrestricted Florida medical license Current DEA license Proven clinical experience Strong clinical judgment and problem-solving skills Ability to develop strong relationships with patients, families, and facility staff Familiarity with electronic medical records and standard medical technology Bilingual skills are a plus Why Geriatric Medicine?
