Role: Skilled Nursing Registered Nurse (RN) providing comprehensive, patient-centered care in the home setting. Key Responsibilities Deliver skilled nursing care in compliance with professional, regulatory, and organizational standards. Conduct thorough patient assessments and ongoing evaluations to determine needs and guide care plans. Develop, implement, evaluate, and revise individualized care plans collaboratively with physicians, patients, families, and the healthcare team. Follow physician orders, coordinate with therapy services, community agencies, and other providers. Apply clinical judgment and critical thinking to make independent nursing decisions. Educate patients, families, and caregivers on disease management, medications, treatments, and health promotion. Maintain accurate clinical documentation and communicate patient status effectively. Participate in on-call rotations, Quality Improvement activities, staff meetings, and training. Qualifications & Skills Proficiency with Microsoft Office; experience with EMR preferred. Strong organizational, communication, critical thinking, and time management skills. BSN preferred or to be obtained within five years; current Ohio RN licensure. BLS certification from AHA required. Minimum of one year of in-hospital or home care nursing experience; interdisciplinary collaboration experience preferred.
