Discharge & Care Coordination Case Manager

Duke Clinical Research Institute

Duke Health is seeking a dedicated Case Manager at Duke Health Lake Norman Hospital in Mooresville, NC. You will coordinate discharge planning and care transitions across the continuum of care, ensuring appropriate utilization of resources and compliance with CMS and other agencies. The role requires BSN or MSW, current NC RN licensure, and 3 years of case management experience; CCM/ACM/ANCC certification within 3 years of hire is expected.
