Primary Care Physician

ChenMed is a physician‑led primary care organization dedicated to caring for older adults. The Primary Care Physician (PCP) will manage a panel of up to 450 patients, focusing on outcomes, cost‑effectiveness, and building trust with patients. The PCP in our organization demonstrates: Accountability for outcomes: The PCP demonstrates accountability for outcomes, strong clinical care, and cost‑effectiveness for each patient in their panel of up to 450 patients, influencing patient outcomes through trust and behavior change. Coaching for health: The PCP acts as a health coach, helping patients set short and long‑term health goals, partnering with patients to work toward those goals, and frequently following up to improve health outcomes. Simplifying for action: The PCP simplifies and prioritizes appropriately so that behavior change is more actionable for patients and for leading their care teams toward performance goals. We are an outcomes‑focused, value‑based organization. For their patient panel, the following metrics are regularly measured to help PCPs become successful and reach partnership status: patient admissions per thousand, use of 18–21 appointment slots per day (new patients count for 2 slots, follow‑ups for 1 slot), CGCAHPS (patient experience), clinical gaps closures, and medical cost measures. Each PCP will have goals for these metrics and will be expected to work toward those targets with center and market leadership as well as their care teams.

ESSENTIAL JOB DUTIES/RESPONSIBILITIES:

The PCP independently provides care for patients with acute and chronic illnesses encountered in older adults. The PCP takes full accountability for patient care and outcomes, seeking specialist consultation when needed while remaining involved in detailed patient care. The PCP engages with the hospitalist whenever one of their patients is hospitalized, regardless of the hospitalist’s affiliation. The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs. The PCP leads their care team consisting of a care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office. For patients unable to come to the office—whether in hospital, SNF, LTC, or homebound—the PCP engages with the transitional care team and other resources available in the market. PCPs actively participate in center management, covering for other providers as needed, and may recruit patients and additional providers for the company. Performs other duties as assigned and modified at the manager’s discretion.

KNOWLEDGE, SKILLS & ABILITIES:

Availability and accessibility for patients to build trust, staying open for walk‑in visits and answering calls and messages promptly. Service orientation, providing care that they would want for a family member or for themselves to each patient. Evidence‑based medicine, staying updated on clinical, nutritional, and lifestyle‑based interventions to improve outcomes. Physician leadership, continuously developing and improving leadership skills for patients, team, center, and company. Quality focus, striving to achieve high quality outcomes and quality measures while continually improving practice and center operations. Influence, competently and compassionately guiding patients, teams, and themselves to achieve optimal outcomes. Self‑care, managing personal physical, mental, social, and spiritual well‑being to provide optimal patient care. Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and other company software. Willingness to travel locally as needed, nationally for initial training (2 weeks), and occasionally regionally or nationally for recruiting or training. Fluency in English, verbal and written; additional language fluency may be required for some centers. Use and exercise of independent judgment.

PAY RANGE:
