Whether you're seeking full-time or part-time work, this is your opportunity to become part of a supportive, professional, and mission-driven team dedicated to high-quality resident care.Located in Porterville, CA, our facility is known for delivering exceptional rehabilitation and skilled nursing services in a collaborative environment where patients and staff thrive.Position SummaryThe Staff Physical Therapist is responsible for evaluating and treating patients, collaborating with healthcare team members, documenting services, and participating in staff training and education.Key ResponsibilitiesEvaluate patients within 24 hours (or 1 business day) of physician referralDevelop and implement individualized treatment plans with physician approvalDeliver treatment in accordance with physician orders and PT best practicesSupervise Physical Therapy Assistants in accordance with state regulationsMaintain detailed, accurate, and timely documentation of all patient careParticipate in interdisciplinary meetings including Care Conferences and Utilization ReviewsProvide caregiver education and training for post-discharge careRecommend and assist with ordering of durable medical equipment (DME)Ensure all department equipment is maintained in good working conditionConduct in-service education for facility staffQualificationsValid California Physical Therapy License (Required)Current CPR Certification (Required)Experience in geriatrics or rehabilitation medicine preferredStrong communication, documentation, and interpersonal skillsAbility to relate effectively with patients, families, and interdisciplinary teamReliable transportation and/or ability to relocate to Porterville, CAWe OfferCompetitive pay starting at $60/hourFull benefits package for eligible employees, including:401(k) with employer matchMedical, dental, and vision insurancePaid time off (PTO)Supportive team culture and ongoing trainingOpportunities for career growth and leadershipIf you're passionate about helping others regain their strength and independence in a caring, team-oriented setting, apply today to join Sequoia Transitional Care in Porterville.
