Job Duties:

• Assist individuals with personal care and self-help skills
• Support transportation and community-access needs
• Prepare meals and provide dining assistance
• Maintain a safe, clean, and comfortable living environment
• Monitor individuals' overall health, safety, and well-being
• Encourage independence and participation in daily activities
• Provide support within intermediate care facilities, community training homes, and supervised living programs
• Document services and daily activities in an electronic record-management system
• Communicate professionally with individuals, coworkers, and support teams

Requirements:

• Compassionate, dependable, and patient approach
• Strong verbal and written communication skills
• Ability to provide personal and residential care
• Basic computer skills for electronic documentation
• Ability to follow individualized support and safety plans
• Strong attention to health, safety, and accountability
• Ability to work independently and as part of a care team
