Health Aide

Position Summary Full time position in the PACE Community Care department located at 155 Addison Street. Responsibilities include providing in‑home assistance to Neighborhood PACE participants with activities of daily living (ADLs) and instrumental activities of daily living (IADLs). The health aide must work independently, solve problems efficiently, seek guidance when needed, and show respect for the participant’s home and community. May also be assigned to other supervised settings such as the PACE Center, as needed. Shift: 7:00 am-3:00 pm, Monday-Friday with every other weekend and holiday rotation. Essential Duties and Responsibilities Consistently monitor participant ADL/IADL care plans and follow the plan of care as stated. Document care provided on participant flow sheets. Check the health aide voicemail at the beginning of each shift and every two hours until the end of the shift. Report any abnormal findings, alterations in participant status, and/or care refusal to the team promptly by calling the day center line or Nurse On Call when applicable. Assist participants with ADLs per assignment, as defined in the ADL/IADL care plan and as needed, e.g., when a participant requests assistance with toileting. Accompany participants to appointments as assigned. Carry out simple treatments under the direction of nursing staff, including simple catheter care and skin assessments during personal care. Qualifications and Requirements High school graduate or equivalent. Current certification as a H.H.A. or C.N.A. from an approved program (e.g., Red Cross). Must demonstrate competencies before providing participant care. Minimum of one year of experience working with a frail or elderly population. Prior experience as a health aide, personal care assistant, or a similar role. Vehicle to travel from home to home preferred. Excellent customer service and communication skills; bilingual a plus. Physical capability to provide ADL and mobility assistance to all PACE participants. Pay: $22/hr - $31/hr based on experience.
