Transitions Care is seeking a compassionate Registered Nurse Admissions to complete hospice admissions and initiate care plans for patients with serious or terminal illness. In this field-based role, you will perform comprehensive assessments, start individualized hospice plans of care, and educate patients and families on services, symptom management, and end-of-life options.
