Clinical Care Coordinator

Community Bridges, Inc. Clinical Coordinator Job Summary Provides clinical and administrative supervision, oversight, training, and mentoring for Case Managers and Rehabilitation Specialists on an assigned team. Ensures clinical quality and productivity standards are met and exceeded, and that assessments and treatment plans are developed and implemented for each patient on the team. Responsibilities

• Supervise and mentor staff, providing clinical oversight and ensuring adherence to quality standards.
• Develop and review assessments and treatment plans for patients.
• Ensure productivity and clinical quality goals are met.

Qualifications Bachelor’s degree in a behavioral health field or related education, training, or experience. At least 1 year of experience working with the SMI population in multiple settings and levels of care, including both mental health and substance abuse. At least 1 year in a behavioral health position providing outreach and engagement activities. Ability to pass criminal background check for jail visitation/clearance. Valid Arizona Driver’s License with 39‑month clear MVR. Behavioral Health Technician (BHT) certification. CPR/First Aid certification. Completion of Crisis Prevention Institute (CPI) training.
