Acute Care Nurse Practitioner

Phoenix, AZ | Full-Time | Onsite

We’re looking for a compassionate and experienced Acute Care Nurse Practitioner to join a collaborative care team dedicated to providing high-quality care to older adults. This role is ideal for a provider who enjoys building meaningful patient relationships while delivering timely, comprehensive care.

What You’ll Do

• Provide primary and acute care, including sick visits, walk-ins, hospital follow-ups, and urgent needs.

• Evaluate, diagnose, treat, and document patient care.

• Partner closely with physicians, RNs, medical assistants, and other care team members.

• Coordinate care with patients’ primary care providers to ensure continuity.

• Support patient outreach, quality improvement, and clinical initiatives.

• Help create a welcoming, efficient, and positive patient experience.

• Participate in occasional after-hours, weekend, or holiday coverage as needed.

• Travel between nearby locations when required.

What We’re Looking For

• Active Nurse Practitioner license in the state of employment.

• 1+ year of experience in primary care, internal medicine, geriatrics, or family medicine.

• Current BLS/ACLS certification .

• Strong clinical, assessment, diagnostic, and documentation skills.

• Experience caring for older adults with complex or multiple medical conditions.

• Excellent communication and interpersonal skills.

• A compassionate, team-oriented approach and genuine commitment to patient care.
