Team Nurse Rooming Pembroke

$32.24 - $55.32 per hour

When you join the growing BILH team, you’re not just taking a job, you’re making a difference in people’s lives. We are looking for an RN or LPN to join our Pembroke family practice. The schedule is Monday‑Friday, 8:30 a.m.–5:00 p.m., with no weekends, nights or holidays. Job Description Primary Responsibilities Serve as a clinical resource, providing high‑quality patient care in a team‑based model within an outpatient family practice. Coordinate and facilitate the quality clinical management of a broad outpatient population across the continuum of care; collaborate with providers to identify preventive care, evidence‑based chronic disease management, and care‑coordination needs; provide general and specific health education; work directly with patients to identify and monitor self‑management goals. Collaborate in care with community‑based providers; follow up on emergency department visits and inpatient discharges; assist providers with exams, treatments and procedures as needed. Provide skilled telephonic triage and assessment, education, and coordination of healthcare and community resources. Perform direct patient care, including Annual Wellness Visits and Chronic Care Management for BIDHC patients. Responsible for INR management and patient teaching for those on Coumadin using a standardized protocol. Administer vaccines and medications by parenteral, oral, rectal or nasal routes as ordered; provide education regarding the administered medication. Carry out nursing activities such as wound care and point‑of‑care testing, as directed by the provider; document all aspects of patient care accurately and in a timely manner; schedule tests and referrals for specialty care. Monitor and ensure follow‑up of abnormal tests and imaging results. Oversee inventory and ordering management of clinical supplies and equipment; ensure safe, accurate and clinically appropriate collection, storage, and processing of biologic specimens. Provide oversight, delegation and support to unlicensed staff—including medical assistants, certified nursing assistants, and nursing students—giving ongoing education and training; keep practice staff updated on clinical knowledge through consultation, literature review, and formal or informal education, including CEUs. Ensure that patient health information is used only for treatment, payment and operations, and report adverse events and near misses promptly to the BIDHC Risk Management Department. Required Qualifications Graduate from an accredited nursing program (Bachelor’s degree preferred). Registered Nurse in Massachusetts (RN) or LPN license; Basic Life Support certification required. 1–3 years of related work experience (new graduate RNs considered). CPR certification. Strong interpersonal, patient communication and education skills. Advanced proficiency with Microsoft Office (Outlook, Word, Excel, PowerPoint, Access) and other web‑based applications. Competencies Decision Making – make decisions guided by general instructions and practices, propose solutions for moderately complex problems. Problem Solving – handle varied problems, analyze situations, use observation, knowledge and precedent. Independence of Action – follow procedures, set priorities, seek assistance when needed; work monitored by a supervisor. Written Communications – communicate clearly and effectively in written English. Oral Communications – speak English and communicate effectively with staff, patients and families. Knowledge – demonstrate full working knowledge of standard practices, procedures and policies. Team Work – collaborate in small teams to improve operations, offering ideas and respecting teammates. Customer Service – provide high‑level service to patients, visitors and staff; remain calm in stressful situations. Age‑Based Competencies Must be competent to provide patient care to all age groups: young adults (16–30 years), middle age (30–60 years), and elderly (60 + years). Social/Environmental Requirements Requires periods of close attention with no interruption, concentrated effort up to 4 hours without break. Requires constant response to changing circumstances and using new information to adapt quickly. Potential exposure to adverse environmental conditions and infectious or bloodborne pathogens; daily protective equipment required (respirator, earplugs, mask, gloves, eyewear). Health‑care worker status varies per department; includes face‑to‑face contact within three feet for at least 5 minutes with physicians, nurses, assistants, etc. Sensory Requirements Close work (paperwork, visual examination); color, visual clarity and perception at 20 ft and below 3 ft; sensitivity to hot/cold, sharp/dull, smell, conversation; monitoring equipment and telephone use. Physical Requirements Medium manual tasks – lift up to 50 pounds occasionally or 20 pounds frequently; stand and move throughout shift. Requires constant sitting, walking, endurance for up to 3–4 hours without a break, frequent standing, bending neck and waist, power grasping, fine manipulation, pushing/pulling with both hands, keyboard use, reaching above and below shoulder height. May require pushing a wheelchair or wheeled bed with a patient up to 250 pounds with assistance; or repositioning a patient up to 100 or 250 pounds with or without assistance.
