$40.00 per hour | Flexible Day & Overnight Shifts

Affirmed Home Care is growing in New Haven and is seeking dedicated Licensed Practical Nurses to deliver high-quality, one-on-one care in a home setting. This is an excellent opportunity for LPNs who prefer a more personalized approach to patient care outside of traditional facility environments.

Your Responsibilities

• Provide direct nursing care in a home environment
• Assist with daily care needs and monitor patient condition
• Communicate effectively with care teams and families
• Ensure a safe, comfortable, and supportive setting for clients

Requirements

• At least 1 year of recent LPN experience
• Active Connecticut LPN license
• Current BLS certification
• Authorized to work in the United States
• Reliable transportation and valid driver’s license preferred
• Physical exam within the past year
• PPD or QuantiFERON within the past year, or chest X-ray within five years if previously positive
