NATIVE HEALTH in Phoenix seeks a Community Health Worker (CHW) focused on Tribal Practices to educate on prevention and holistic wellness for Indigenous people in Maricopa County. You will bridge the Community Health, Medical, and community access to services. You will report to the Indigenous Wellness Coordinator and collaborate with clinical and outreach teams.
