As an LVN with Los Robles, you will provide care and support to patients during their final stages of life./ppIn this role, our ideal candidate will have knowledge and skill in supporting patients through end-of-life care. This includes direct patient care, educating families, providing care aligned with patients values, and providing support for patients and families. We seek a compassionate nurse with excellent interpersonal skills who possesses a thorough understanding of hospice care./ppPosition Availability and Area Coverage :/ppWe are seeking a candidate who is available for on-call work, and is able to visit our patients in the Gardena, CA area./ph3Responsibilities :/h3ulliProvide care in various community settings (ie. SNF, AL, Home and RCFE)/liliAdminister and educate caregivers about medications medical supplies/equipment/liliProvide skilled nursing visits for patients (ie. symptom visits, wound care etc.)/liliCollaborate with interdisciplinary team members (RN, MSW, chaplain, HHA, volunteers)/liliTimely charting of all visits/liliWork closely with collaborating RN case manager/li/ulh3Qualifications and Skills :/h3ulliGraduate from an accredited Licensed Vocational Nurse program/liliMust possess a current, unrestricted California LVN license and CPR certification/liliOne year of nursing experience/liliMinimum 1 year experience specifically in home health and/or hospice or skilled nursing facility/liliExperience with wound care, phlebotomy, and medication teaching/liliMust be able to pass required background check/liliExcellent verbal/written communication skills/liliCommunicates effectively with patients, families, and team members/liliCompassionate, professional, responsible/liliKnowledge and practice of infection control guidelines/liliMust comply with health screening and immunization requirements/liliFamiliarity with the use of Electronic Medical Records (EMR)/liliFollow professional standards guidelines for the organizations/liliMust have valid driver license, reliable transportation source and proof of auto insurance/liliBilingual a plus (our patients speak many languages!)/liliValid CA drivers license/liliCurrent CPR certification and current professional certification./li/ulpWhy work with us?/ppWe have a team approach that is focused on our success in achieving a great work environment and excellent patient outcomes.
