Physician - Geriatrics Home Based Primary Care

Overview The New Mexico VA Health Care System is seeking a Medical Director (Physician) for the Home Based Primary Care Program to provide comprehensive home-based primary care to frail - chronically ill Veterans. The physician delivers longitudinal care, coordinates with an interdisciplinary team, and supports quality, safety, and performance goals for Veterans with complex medical needs. Qualifications United States Citizenship (Non-citizens may be appointed only when recruitment of qualified citizens is not possible). Doctor of Medicine or equivalent degree in allopathic or osteopathic medicine, obtained from an VA-approved school at the time of completion. Current, full and unrestricted license to practice medicine or surgery in a State, Territory, Commonwealth, or the District of Columbia. Residency training approved by the Secretary of Veterans Affairs in an accredited core specialty program leading to board‑certification eligibility. Completed one of the following residency pathways: ACGME or AOA accredited core residency program. At least one year of ACGME or AOA‑approved post‑medical‑school training followed by two years of independent practice under a full license. Non‑US residency training followed by a minimum of three years of verified independent practice in the United States, with fellowships counted toward this requirement. Resident physicians currently enrolled in ACGME/AOA accredited programs may be appointed as Physician Resident Providers (PRPs) on an intermittent basis, provided they are fully licensed physicians. Proficiency in spoken and written English. Preferred: Board Eligible or Board Certified in Geriatrics; at least two years of outpatient geriatrics experience; supervisory experience of providers. Physical Requirements: light to moderate lifting (15‑44 lbs.), walking up to 2 hours, standing up to 2 hours, and other standard visual, hearing, and motor skill criteria as outlined in the job announcement. Duties Assume overall medical responsibility for assigned HBPC patients, including input on admission criteria. Serve as medical director, ensuring clinical metrics and quality standards are met. Collaborate with the HBPC Program Director and GEC Leadership to deliver high quality care to veterans, including oversight of satellite HBPC sites in Santa Fe, Gallup, and Farmington. Adhere to the National VHA HBPC Directive and related procedures. Determine need for consultation with medical-surgical, psychiatric, or other subspecialty clinics. Coordinate hospital admission and other transitions of care. Conduct home visits for medical evaluation and treatment when clinically indicated. Perform virtual visits using VA‑approved platforms. Provide administrative support, including completion of physician‑required orders and forms for HBPC patients. Plan and direct educational and clinical experiences for medical students, residents, and fellows assigned to the HBPC program. Lead continuous performance‑improvement initiatives for the HBPC program. Participate in interdisciplinary care meetings to develop individual plans of care for primary‑care provider‑assigned panels of patients. Support caregivers in continuing to provide care. Coordinate processes with team members to maximize efficiency and improve clinical outcomes. Assess patients' continuing need for HBPC care and plan for discharge when indicated. Utilize VHA and community resources as appropriate. Collaborate with GEC care teams—including staff physicians, residents, nurse practitioners, nurses, clerical staff, and other support staff—to deliver patient‑centered care. Participate in lean processes to improve patient care services with changing needs and staffing levels. Engage in peer review and quality‑improvement initiatives as needed. Perform chart review, Focused Professional Practice Evaluation (FPPE), and Ongoing Professional Practice Evaluation (OPPE). Ensure provider competencies are maintained and documented with the Medical Staff Office and Human Resources. Serve on clinical and administrative committees, attending at least 50% of regularly scheduled Medical Staff meetings, as directed by the Chief of Geriatrics and Extended Care. Perform other duties as assigned. Work Schedule: 8:00 am to 4:30 pm, Monday to Friday. Benefits Competitive salary with annual performance bonus and regular salary increases.
