Hospice Social Worker — Family & Caregiver Support

JourneyCare in Oak Park, IL is seeking a Social Worker to perform psychosocial assessments for hospice patients and families in their homes or community settings. You will ensure needs are addressed in the plan of care and coordinate with care teams. Responsibilities include counseling patients and families, documenting services, assisting with applications for SSI, hospice, Medicaid, and connecting to community resources.
