Position Overview

Dedicated Support Network is seeking a highly skilled and detail‐oriented Program Specialist to support Individuals with intellectual disabilities in accordance with Chapter 6400 regulatory requirements. The Program Specialist plays a critical role in assessment, planning, service coordination, ISP development, and ensuring high‐quality, person‐centered supports. One Program Specialist is assigned for every 12 Individuals served. This position requires strong clinical judgment, exceptional documentation skills, and the ability to coordinate multidisciplinary teams while ensuring regulatory compliance and individualized care.

Assessment & Planning

• Coordinate and complete high‐quality, accurate assessments for Individuals.
• Provide assessments as required by regulations.
• Participate in individual plan development, team reviews, and implementation in accordance with Chapter 6400.44.
• Ensure assessments and ISPs contain consistent information and that services align with both documents.
• Attend ISP meetings and fulfill the role of plan lead for development, annual updates, and revisions.
• Review ISPs, updates, and revisions for content accuracy and report discrepancies to the Supports Coordinator (SC) and plan team members.
• Recommend revisions to services or outcomes in the ISP as needed.
• Implement the ISP as written and ensure all services are delivered according to regulatory requirements.
• Supervise, monitor, and evaluate services provided to Individuals.
• Review, sign, and date monthly documentation of Individual participation and progress toward outcomes.
• Report changes in Individual needs to the SC and plan team members.
• Coordinate all services provided to Individuals, ensuring continuity and quality.
• Support Individuals’ integration into the community and involvement with family and friends.
• Ensure individualized approaches to supporting communication and family engagement.
• Review the ISP with Individuals as required and document the review per regulations.
• Provide ISP review documentation to the SC and plan team members as required.
• Inform plan team members of the option to decline ISP review documentation.
• Maintain compliance with all Chapter 6400.44 requirements and DSN policies.
• Coordinate training for direct service workers on health and safety needs relevant to each Individual.
• Develop and implement provider services as required by regulations.

Qualifications

Candidates must meet one of the following educational and experience requirements:

• Master’s degree from an accredited college or university and 1 year of direct experience working with individuals with intellectual disabilities.
• Bachelor’s degree from an accredited college or university and 2 years of direct experience working with individuals with intellectual disabilities.
• Associate degree or 60 credit hours from an accredited college or university and 4 years of direct experience working with individuals with intellectual disabilities.

Additional Requirements

• Valid Pennsylvania Driver’s License.
• Strong knowledge of ISP processes, regulatory compliance, and person‐centered planning.
• Excellent communication, documentation, and organizational skills.
