Job Summary
We are seeking a Nurse Practitioner to join our team. In this role, you will primarily be creating and managing a plan of care for individual wound care patients. You are a NP who operates with great attention to detail and a big heart. You take pride in creating personalized care plans for patients and are able to observe and communicate effectively in your patient's best interest. To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as being able to work autonomously. 

Responsibilities 

• Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans 
• Conduct thorough patient evaluations, including physical examinations and health histories, to establish appropriate treatment plan.
• Educate patients and their families on wound management techniques, nutrition for healing, and lifestyle modifications to promote long-term health
• Perform specialized wound care procedures
• Utilize electronic health record (EHR) systems like WoundAlly for documentation , medical records management.
• Administer treatment plans and monitor patients for improvement  
• Document any changes in patient conditions and report on changes 
• Maintain accurate records of patient care and discuss observations with wound care team  
• Visiting patients home, facilities, etc. in the Los Angeles County, Orange County, Inland Empire and surrounding areas.
• Diagnose and treat acute and chronic wounds.
• Prescribe medications and manage treatment plans.

Qualifications

• Current Nurse Practitioner license in California.
• Previous experience as a NP or Registered Nurse is preferred
• Ability to work independently and as part of a team.
• Excellent interpersonal and client care skills
• Valid driver’s license/Registration
• First aid/CPR certification required 
• Current DEA and professional liability insurance.
• Commitment to continuing education and professional growth.

Work Location: On the road / Office
