Administer medications and monitor patient responses. Collaborate with healthcare team to develop patient care plans. Participate in ongoing professional development and training. Monitor and report changes in patient conditions to the nursing team. Document patient progress and maintain accurate health records. Ensure compliance with healthcare regulations and standards. Utilize performance metrics to improve personal and team outcomes. Assist in the training and mentoring of new staff. Engage with patients and families to provide emotional support. Maintain a clean and safe environment for patients. Participate in team meetings to discuss patient care strategies. Experience/Qualifications Valid LPN license with experience in long-term care settings. Commitment to ongoing professional development and training. Ability to provide compassionate support to patients and families. Ability to monitor and report changes in patient conditions promptly. Experience in documenting patient care accurately and timely. Strong collaboration skills to work with healthcare teams effectively. Knowledge of healthcare regulations and compliance standards. Experience in utilizing performance metrics to enhance care outcomes. Willingness to assist in training and mentoring new staff members. Ability to maintain a clean and safe environment for patients.
