Home Health Physical Therapist (PT)
Schedule: Monday–Friday, 8:00 AM–5:00 PM
No Weekends | No On-Call
We're looking for a compassionate and experienced Home Health Physical Therapist (PT) to join our team and provide exceptional rehabilitative care to patients in the comfort of their homes. This role offers a predictable weekday schedule, competitive mileage reimbursement, and the opportunity to make a meaningful impact on patients' independence and quality of life.
Position Details
- Schedule: Monday–Friday, 8:00 AM–5:00 PM
- No weekends or on-call responsibilities
- Productivity Requirement: 28 points/units per week
- Mileage Reimbursement: $0.54 per mile
- License Required: Active West Virginia Physical Therapist license or Compact PT license
Territory
This position serves patients in:
- Raleigh County
- Wyoming County
Responsibilities
- Perform comprehensive physical therapy evaluations based on physician referrals.
- Develop individualized treatment plans with measurable goals to maximize patient independence and functional mobility.
- Provide skilled physical therapy services in patients' homes.
- Regularly evaluate patient progress and modify treatment plans as needed.
- Educate patients and caregivers on exercises, mobility techniques, safety, and injury prevention.
- Collaborate with physicians, nurses, case managers, and interdisciplinary care teams to coordinate patient care.
- Recommend and train patients in the use of adaptive equipment to improve function and safety.
- Complete accurate and timely clinical documentation, evaluations, treatment notes, and billing documentation.
- Ensure compliance with company policies, HIPAA regulations, and all state and federal guidelines.
- Supervise Physical Therapist Assistants (PTAs) and Physical Therapy Aides in accordance with state practice regulations.
- Participate in continuing education, staff meetings, and professional development activities.
Qualifications
- Active West Virginia Physical Therapist (PT) license or Compact PT license.
- Home Health experience required.
- OASIS experience required.
- HomeCare HomeBase (HCHB) experience preferred.
- Strong clinical assessment and treatment planning skills.
- Excellent communication and patient education abilities.
- Ability to work independently while managing a designated territory.
