For off-site visits, this will involve driving to the patient's residence which can be Assisted living, SNF, or personal residence where usually with a nurse from our team will join the provider for the visit. We serve exclusively adults over the age of 55 as a PACE program (Program of All Inclusive Care for the Elderly) which means you'll be working with a 11 person interdisciplinary team. Most patient management is handled through the EMR with other team members coordinating visits and care in the community.
