Job Details Overview:

As a Full-Time LPN with us, you'll be the ray of sunshine in our patients' lives, providing direct care and support under the guidance of our registered nurses. Specific area of coverage will be Chesapeake, Suffolk and Isle of Wight territory.

Responsibilities:

Your responsibilities will include:

Providing direct patient care as outlined in the State Nurse Practice Act.

Implementing care plans initiated by our clinicians.

Documenting patient care accurately and promptly.

Educating patients and their families/caregivers on diagnoses and care plans.

Collaborating with the team to coordinate home health services.

Using equipment and supplies effectively and efficiently.

Embracing opportunities for personal and professional growth.

Assisting with other duties as assigned by our Registered Nurses.

Qualifications:

We're looking for LPNs who shine bright with the following qualifications:

Graduate of an accredited practical nurse or vocational nursing program.

Preferably, at least one year of nursing experience.

Currently licensed as an LPN or LVN in the State.

Demonstrates excellent verbal and written communication and organizational skills.

Must be a licensed driver with a reliable and insured automobile.

Functional Abilities:

Ready for lots of walking, standing, and occasional lifting.

Able to bend, stoop, and move with ease.

Good visual acuity and hearing.

Can read 12-point or larger type.
