Special Operations Case Manager / Nurse Case Manager (Hampton Roads, VA) Location: Hampton Roads, VA. Relocation provided. Responsibilities Collect, organize, record, and communicate data relevant to primary health assessments including detailed medical history to develop time-sensitive treatment plans. Assess patients via telephone using established protocols, providing appropriate and cost-effective care. Establish priorities for patient care, monitor and evaluate progress toward goals to provide coordinated, efficient, effective healthcare to beneficiaries. Oversee discharge planning to ensure timely and sequenced patient care transitions. Assist clinicians in identifying patient needs for referrals to appropriate healthcare providers or facilities. Negotiate, write, finalize, and administer product/service agreements ensuring compliance with care goals. Apply utilization review criteria and recognize quality and risk management issues to improve practice and ensure compliance. Use VA/DOD Clinical Practice Guidelines to ensure evidence-based standardized care for military beneficiaries. Maintain knowledge of healthcare financial environment, fiscal stewardship of population health resources. Employ research tools to critically evaluate practices, opportunities, and outcomes. Manage information systems and technology for individual or population health, improving practice and communicating outcomes. Conduct data collection, tracking, rendering, and analysis using computer applications. Design and present computer-generated outcome analysis for peer and higher-level educational programs or project briefings. Understand pharmaceuticals, their effects, side effects, and complications to gauge patient recovery impact. Required Education, Experience & Skills U.S. citizen with a Secret or Top Secret clearance, or the ability to obtain one. Current, full, active, unrestricted license as a Registered Nurse or Licensed Clinical Social Worker in any U.S. state, D.C., Guam, Puerto Rico, or U.S. Virgin Islands. Minimum of two (2) years of nursing or case management experience; must be certified as a case manager by a recognized certifying organization (e.g., Commission for Case Management Certification, American Case Manager Association, American Nurse Credentialing Center) or obtain certification within the first year. Demonstrated knowledge of patient education principles and management of complex medical, psychosocial, and financial problems. Ability to seek and incorporate feedback from peers, colleagues, clients, and research to expand knowledge and improve performance. Skills in consulting with healthcare providers and negotiating external services and products. Maintenance of clinical practice privileges and credentialing at local MTF may be required. Fluent oral and written English communication skills for interactions with peers, agencies, vendors, providers, and MHS stakeholders. Team coordination, facilitation, and communication skills to support information exchange, goal achievement, and systemic change. Knowledge of local, state, and federal laws, regulations, professional ethics, and practice standards. Current Basic Cardiac Life Support (BCLS) certification; proof of all training and documentation required. Desired Qualifications Previous military background or experience.
