RN Case Manager – Pediatric Home Health (Field-Based) $77,000–$80,000/year |Monday–Friday Company Vehicle Provided|Travel Time Included in 40hr Workweek Are you an RN who enjoys pediatric care but is looking for a more predictable schedule and meaningful one-on-one patient relationships? CareGivers America – NEPA is hiring a full-time RN Case Manager to support pediatric patients and families throughout Monroe, Lehighton, & Northampton County .You’ll serve as a trusted clinical partner to families, coordinate care, and help children receive high-quality support from the comfort of theirhome. Your Role as aPediatric RN Case Manager: Complete patient assessments, including OASIS data collection Develop and oversee pediatric care plans with physicians and families Supervise and support LPNs and caregivers Coordinate care with families and interdisciplinary teams Ensure clinical documentation is accurate, timely, and compliant Participate in quality improvement and staff education Promote safe, high-quality care in the home Territories: Monroe, Lehighton, & Northampton County Office Address: 718 S.
