Requirements: CM acute experience

Responsibilities:

• Manage cases for acute patients in a hospital setting.
• Collaborate with healthcare team to develop patient care plans.
• Document patient progress using Meditech and Midas EMR systems.
• Ensure compliance with hospital policies and procedures.
• Communicate effectively with patients and staff to coordinate care.
