Travel Pt Home HealthTravel PT Home Health jobs in Bucks County, PA and Northeast Philly are available for 13-week contracts, Monday through Friday with 8-hour days and 40 hours expected weekly. You must have a Pennsylvania license, home health experience, and Start of Care experience. You will visit 5 to 6 clients daily and document using Homecare Home Base. Mileage is reimbursed at $0.72 per mile. Your responsibilities include performing in-home physical therapy evaluations, developing patient-centered care plans, delivering skilled therapy to improve mobility and safety, and educating patients and caregivers on exercises and equipment use. You will collaborate with a multidisciplinary team and coordinate care with nursing, occupational therapy, and social work.
