Travel LCSW - Full Time Contract

Make a meaningful impact as a Licensed Clinical Social Worker. We are seeking an experienced LICSW who will advocate for patients and families, assess psychosocial needs, coordinate discharge planning, and connect patients with community resources in an acute care hospital setting in Hyannis, MA.

Minimum Qualifications

• Master’s degree in Social Work (MSW) from an accredited program.
• State licensure as a Licensed Independent Clinical Social Worker (LICSW) or L.C.S.W. in good standing with the state licensing board.
• Experience conducting comprehensive psychosocial and risk assessments, including mental health and substance use evaluations.
• Experience developing discharge plans and coordinating care with interdisciplinary teams and community agencies in an acute care or hospital setting.
• Proficiency with electronic health records and documentation standards.
• Ability to communicate effectively in English, both verbally and in writing.
• Crisis intervention and supportive counseling skills.
• Required certifications: CPI (Crisis Prevention) / MAB within one month of hire and maintained annually.
• Preferred: Acute care hospital social work experience.

Assignment Details

• Facility type: Hospital.
• Shift: Days.
• Shift hours: 08:00–16:00.
• Work schedule: 5 shifts per week, 8 hours per shift (40 hours per week).
• Assignment duration: 26 week travel assignment.
• Candidate must be able to perform clinical assessments, discharge planning, and liaison duties across clinical units as assigned.

Role Summary and Core Responsibilities

• Perform comprehensive psychosocial assessments for patients and families to identify needs related to home environment, family dynamics, mental health, substance use, economic and cultural factors.
• Conduct risk assessments and implement interventions for at-risk populations including elders, children, persons with disabilities, and survivors of abuse or domestic violence.
• Collaborate with interdisciplinary treatment teams and the emergency psychiatric team to develop and implement safe, timely discharge plans and appropriate referrals to community resources and higher levels of care.
• Provide crisis intervention, advocacy, supportive counseling, and education to patients and families regarding treatment plans and third-party payer processes.
• Facilitate placement decisions, procurement of community services and equipment, and coordination with external agencies and long-term care facilities as indicated.
• Document all assessments, interventions, and care plans in the electronic health record in accordance with policy.
• Participate in departmental meetings, quality assurance activities, and committee work as requested.
