Qualifications (What You Bring)

• Graduate from an accredited nursing program; a Bachelor’s degree is preferred.
• Active RN license in the state of Colorado.
• At least two (2) years of nursing experience, including long-term care, dementia care, or acute care; home care experience is a plus.
• CPR certification.

Key Responsibilities

The Registered Nurse (RN) is responsible for case management, coordination, supervision, and the delivery of professional and supportive services to clients, utilizing nursing theory and processes in line with the Nurse Practice Act.

• Evaluates client referral information and addresses requests and inquiries as needed.
• Conducts client assessments according to agency standards to ensure effective home care services.
• Independently directs, plans, and initiates appropriate actions in home care situations.
• Performs duties in compliance with agency policies, procedures, and professional standards.
• Completes additional tasks as assigned by the Director of Clinical Services.

We have a dedicated team of nurses, rehabilitation therapists, and home health aides committed to delivering personalized healthcare with compassion and empathy. We specialize in caring for older adults, providing in-home health services across Delta, Montrose, and Ouray counties.

• 2 years of nursing experience, including long-term care dementia care or acute care

• Experience in home care.

• Bachelor’s degree in Nursing or a related field

• CPR/First Aid certification
• Registered Nurse license
