Registered Nurse (RN) Care Manager – Pediatric Home Care
Per Diem | Field-Based | Bronx County, NY
Join a Premier Concierge Pediatric Home Care TeamAffirmed Home Care is seeking an experienced, compassionate, and motivated Registered Nurse (RN) Care Manage r to support pediatric clients throughout Bronx County, NY.
This per diem field-based position is perfect for an independent nurse who values flexibility, autonomy, and the opportunity to build meaningful relationships with patients and families while delivering exceptional one-on-one care in the home setting.

Position Responsibilities- Conduct in-home nursing assessments and case management visits
- Develop, implement, and oversee individualized pediatric care plans
- Obtain and maintain physician orders in accordance with NYS regulations
- Monitor, document, and communicate changes in patient condition
- Perform supervisory visits for home care staff
- Collaborate with families, physicians, and interdisciplinary care teams
- Promote patient safety, comfort, independence, and continuity of care
- Minimum of 2 years of recent RN experience in pediatric home care or acute care
- Strong pediatric clinical experience with:
- Ventilator and tracheostomy care
- G-tube/PEG tube care
- IV infusions
- Wound care management
- Active New York State RN License
- Current BLS Certification
- Recent physical exam (within 1 year)
- Current PPD/QuantiFERON (within 1 year) or chest X-ray (within 5 years)
- Valid MMR lab report
- Valid driver’s license and reliable transportation
- Authorized to work in the United States
- $150 per assessment
- $125 per aide supervision visit
- $55/hour
