Collaborate closely with physicians and interdisciplinary teams to optimize patient outcomes and ensure compassionate, patient-centered care.Key Responsibilities:Create individualized care plans based on medical and nursing diagnoses and patient goals.Deliver skilled nursing services aligned with the care plan, focusing on symptom management and patient comfort.Maintain clear communication with hospice house staff, volunteers, patients, and families regarding patient assessments and care updates.Supervise Home Health Aide services in accordance with policy guidelines.Complete timely and accurate documentation following agency standards.Foster positive relationships with patients, families, healthcare providers, and referral sources.Observe and assess patients' physical, emotional, mental, and spiritual well-being, collaborating with the care team as needed.Adhere strictly to HIPAA regulations to protect patient privacy and confidentiality.Education and Experience:Graduate of an accredited nursing program.Several years of clinical nursing experience preferred.BSN degree is advantageous.Previous hospice or home health experience is highly valued.Certification as a Hospice and Palliative Care Nurse is preferred.Licenses and Certifications:Current Registered Nurse license valid in the applicable state.BLS certification required within three months of hire.Valid driver's license and proof of auto insurance as required.Skills and Attributes:Proficient with MS Office and strong keyboarding skills.Excellent communication and multitasking abilities.Strong integrity, compassion, accountability, respect, and commitment to excellence.Ability to work independently and adapt to changing priorities.Demonstrated leadership and conflict management skills.This role offers a rewarding opportunity to provide exceptional hospice care within a supportive and collaborative environment serving the Central Plains region of the United States.
