Experienced Home Health Physical Therapist (PT)
One Home Health serves patients throughout Joliet and surrounding communities . Multiple service territories are available, and assignments are made based on patient needs, clinician preference, and geographic location whenever possible.
Compensation
Job Type: Full-Time, Part-Time, PRN/Per Diem
Pay: $75.00 - $85.00 per hour Compensation is based on experience, qualifications, and clinical expertise.

Responsibilities
· Perform comprehensive physical therapy evaluations in patients' homes.
· Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence.
· Provide skilled physical therapy interventions according to physician orders and established care plans.
· Assess patients' home environments and recommend safety modifications and assistive devices as appropriate.
· Educate patients and caregivers on exercises, injury prevention, fall prevention, and home safety.
· Collaborate with physicians, nurses, occupational therapists, speech therapists, and other members of the interdisciplinary care team.
· Monitor patient progress and adjust treatment plans based on clinical findings.
· Complete accurate, timely, and compliant clinical documentation.
· Participate in discharge planning and patient outcome evaluations.
Qualifications
Required
· Active Illinois Physical Therapist (PT) license.
· Graduate of an accredited Physical Therapy program.
· Current CPR/BLS certification.
· Valid driver's license and reliable transportation.
· Excellent communication, organization, and time-management skills.
· Ability to work independently in a home health setting.
Preferred
· Home health experience preferred.
· Experience with Medicare home health regulations and documentation requirements.
· Knowledge of OASIS documentation.
· Experience with Axxess software.
· Strong orthopedic, neurological, geriatric, or rehabilitation background.
· Bilingual candidates are encouraged to apply.
