Intake Coordinator

Job TitleCoordinates and assists in the provision of health resources and services to clients.Essential FunctionsAssists families in determining needed resources and services through a screening and triage process.Answers questions and concerns of families and provides service referrals and scheduling.Gathers referral and pre-admission information and completes all pre-admission data entry.Identifies emergent or complex cases and seeks clinical review prior to finalizing the referral.Collaborates with interdisciplinary teams to ensure efficient resource and scheduling services.Utilize Electronic Medical Record and Scheduling software to assess the demand for services and the ability of current service capacities to meet the community's needs.Maintains confidentiality of all clinical and administrative information consistent with agency policy.Education RequirementBachelor's Degree in relevant field, or equivalent experience, required.SkillsStrong skills in customer service, process improvement, data entry, and advanced computer skills, required.Proficiency with EPIC, preferred.ExperienceCustomer service or other relevant experience, required.Physical RequirementsOccasionally: Bend/twist, Climb stairs/ladder, Flexing/extending of neck, Lifting / Carrying: 0-10 lbs, Pushing / Pulling: 0-25 lbs, Reaching above shoulder, Squat/kneelFrequently: Standing, WalkingContinuously: Audible speech, Color vision, Computer skills, Decision Making, Depth perception, Hand use: grasping, gripping, turning, Hearing acuity, Interpreting Data, Peripheral vision, Problem solving, Repetitive hand/arm use, Seeing – Far/near, SittingThe above list of duties is intended to describe the general nature and level of work performed by individuals assigned to this classification.
