CalAIM Care Manager — Community Health Navigator
Eastern Plumas District Hospital
Eastern Plumas Health Care is seeking a Care Manager to provide outreach and direct case management services within a CalAIM Community Care team. You will work with Plumas and Sierra Counties, supporting participants to achieve wellness goals with empathy and non-judgmental care. Under supervision, you will coordinate referrals, track progress, and maintain accurate records.
