Responsibilities
- Assess patients in a home health context and develop individualized PT treatment plans to improve mobility and daily function.
- Deliver PT interventions including therapeutic exercise, manual therapy, gait training, and modality application as appropriate for the home setting.
- Monitor progress, document outcomes, and adjust treatment plans in collaboration with the rehabilitation team.
- Coordinate care with physicians, nurses, and other health professionals across multiple sites within the rural health system.
- Ensure patient safety and apply infection control and safety guidelines during visits; complete required documentation in electronic health records.
- Manage a travel-based caseload, prioritizing patient needs and coordinating equipment or supplies as needed.

Requirements
- Active Montana Physical Therapist license
- Basic Life Support (BLS) certification
- Graduation from an accredited physical therapy program with a bachelor’s, master’s, or doctorate degree in PT
- Certifications such as dry needling or pelvic floor therapy are referenced in the job notes
- Experience using computer applications in a healthcare setting
- Ability to work in a home health/rural setting and travel between sites as required
