Job Summary:  

One of our direct clients is seeking a dedicated Physical Therapist (PT) to join their Home Health team in Kings County, CA/Colorado Springs, CO. The ideal candidate will provide high-quality, patient-centered care, helping individuals restore mobility, improve functional independence, and enhance overall quality of life in a home-based setting. 

Key Responsibilities:  

• Conduct initial evaluations and ongoing assessments in patients’ homes 

• Develop and implement individualized treatment plans based on physician orders 

• Provide skilled therapy services including therapeutic exercises, gait training, balance training, and manual therapy 

• Educate patients and caregivers on home exercise programs and safety techniques 

• Monitor patient progress and adjust treatment plans as needed 

• Complete accurate and timely documentation in compliance with regulatory standards 

• Collaborate with physicians, nurses, and interdisciplinary team members 

• Participate in case conferences and care coordination 

Qualifications:  

• Active Physical Therapist (PT) license 

• DPT or equivalent 

• Experience (home health preferred) 

• Strong clinical, communication, and organizational skills 

• Ability to work independently in a field-based role 

Preferred Qualifications:  

• Prior home health experience 

• Experience with EMR systems (e.g., Homecare Homebase, WellSky)
