Responsibilities

• Provide outpatient and inpatient social work services to identified high-risk patients; assess needs, develop care plans, and coordinate services
• Collaborate with a multidisciplinary team (physicians, nurses, case managers, care coordinators) to ensure a seamless continuum of care
• Conduct discharge planning, utilization review, and assist with admission decisions for ED presentations and other hospital events
• Link patients with community resources, behavioral health and substance use services, and follow through to achieve measurable outcomes
• Monitor patient progress, reassess care plans, and advocate for patient and family engagement to promote safety and self-management
• Document care processes and utilize the electronic health record to support case management and care coordination across settings

Requirements

• Master’s degree in Social Work (MSW)
• Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) with an active, unrestricted license in any U.S. state or territory
• Current Basic Life Support (BLS) certification from the American Heart Association
• Minimum three (3) years of clinical care experience; experience with discharge planning, case management, or utilization review
• Knowledge of electronic health record systems; ability to work effectively in inpatient and outpatient settings
• Ability to collaborate with a multidisciplinary team and work within a health system
