Case Management RN - Patient Care Coordinator
Hudson Regional Hospital
Hoboken University Hosiptal in Hoboken, NJ seeks a Case Manager to oversee admission reviews, resource utilization, and collaboration with the Patient Access team to ensure timely admissions for clinical review.
The role requires a Bachelor’s degree in social work, at least 2 years of related experience, and strong interpersonal and communication skills. Hybrid schedule may apply and EMR documentation must be precise and timely.
