CVS Health is seeking a Health Coach Consultant (Registered Nurse) in Phoenix, AZ. The role engages members through assessment, planning and coaching to promote optimal health and informed decisions. Responsibilities include evaluating member needs, applying clinical guidelines, and supporting behavior change via motivational interviewing and care plans. Requires active RN licensure, 3+ years acute care, and MS Office experience; schedule is weekdays 8am–4:30pm with one weekly evening shift. #J-18808-Ljbffr null
