Care Manager

Role Description The Care Manager is a part-time, working from home based position seeing clients where they live in Pinellas, Pasco and Hillsborogh counties. Responsible for overseeing and coordinating services for older adults and their families. Day-to-day tasks include conducting comprehensive assessments, developing individualized care plans, and monitoring client progress to ensure needs are met. The Care Manager collaborates with healthcare providers, social service agencies, and community organizations, and helps clients access appropriate medical, social, and supportive services. The role involves frequent in-person visits, advocacy on behalf of clients, documentation of care activities, and ongoing communication with families and stakeholders. The Care Manager also provides education, guidance, and support to help clients and families make informed decisions about care and living arrangements.

Qualifications

• Strong Case Management skills, including assessment, care planning, coordination, and follow-up.
• Experience in Elder Care, with understanding of aging-related needs, resources, and best practices.
• Background in Social Services, including familiarity with community programs, benefits, and support systems.
• Relevant knowledge of Medicine or healthcare settings to effectively collaborate with medical professionals.
• Ability to provide Training and education to clients, families, and caregivers on care options and resources.
• Degree in Social Work, Nursing, Gerontology, Counseling, or related field; licensure or certification in a realted health care field or in care management is a plus.
• Excellent communication, documentation, and interpersonal skills, with the ability to work with diverse populations.
• Strong organizational skills, reliable follow-through, and comfort working in an on-site, client-facing environment.
• Valid driver’s license and reliable transportation for local client visits in the above outlined area.
