Triage After Hours RN / Hospice On-Call Registered Nurse

We are currently recruiting for a Hospice RN to join our hospice's after-hours nursing team. The working hours are scheduled on a 7 days on / 7 days off format. During the seven days on, the expectation is you are available if a need arises out outside of normal business hours with one of our patients. If there are no needs, you are free to spend your time how you like, as long you are able to commute to a care need in a timely manner. The position is salaried.
Responsible for managing all aspects of the patient intake process after hours, including managing the members of the intake team, establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and managing the insurance verification and authorization processes. DUTIES & RESPONSIBILITIES 1. Following the 7 days on this position receives 7 days off in a row.2.Conducts patient visits and gathers information for RN Case Manager. 3. Performs treatments, including wound care, as directed by the RN Case Manager. 4. Assists in identifying discharge-planning needs. 5. Reports changes in the patient's physical condition or care needs to PCP. 6. Orders medication, DME and other supplies to be delivered to patient as needed. 7. Participates in quality assessment and performance improvement activities. 8. Works with Admissions Coordinator and admissions team to ensure timely evaluation and admission of patients to hospice, when admission is scheduled outside of normal business hours. 9. With regard to admissions, ensures seamless transition of patients to hospice care by providing direct oversight of patient education and preparation for hospice care, plan of care initiation, and coordination of care with multiple service providers. JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities) 1. RN with current licensure to practice professional nursing in Arizona. 2. Ability to assess patient's needs and direct care to meet those needs. 3. Demonstrates good communications, negotiation, and public relations skills. 4. Ability to assess patient's needs and direct care to meet those needs. 5. Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities. 6. Valid Driver's License. 7. Automobile in good working order. 8. Proof of current automobile insurance.
