Job Details PAY: $50.00 - $140.00 per visit Job Type: W-2 Per Diem Medical Specialty: Home Health Work Location: On the road Availability Our patients need care 7 days a week. You must be able to work at least two to three days each week (or ten productivity units), one weekend a month, and two holidays per year (one winter and one summer). Work Area: City close to you, with a group of patients who need care for that day. Responsibilities Travel to patient’s home to administer services and help maintain independence. Administer at‑home IVs, change dressings, clean wounds, perform in‑depth assessments, and oversee patient treatment plans. Update doctors and the agency about patient health via Synergy Software. Perform an initial, comprehensive assessment; document and upload to Agency Portal. Obtain and document physician orders. In consultation with the assigned Clinical Team Manager, initiate, re‑evaluate, and revise the plan of care regularly. Coordinate care with the Clinical Team Manager, physician, and other home‑care team members, informing them of significant changes in the patient’s condition and needs. Perform re‑certifications, resumption of care, transfers, and discharges as requested. Complete and submit all related documentation. Assess need for services of other team members (PT, OT, ST, MSW, HHA). Perform Home Health Aide supervisory visits per regulatory requirements. Demonstrate knowledge of regulatory requirements. Qualifications Visiting RN for Home Health experience: at least 1 year (Required). General nursing experience: at least 1 year (Required). Current California RN license (Required). BLS certification from the American Heart Association or American Red Cross (Required). NSO Nursing License Insurance (Required). IV/Phlebotomy certification (Required). #J-18808-Ljbffr
