Our client is currently seeking a Case Manager RN Schedule: Monday - Friday | 8:00 AM - 5:00 PM (Approx. 20 hours/week)

We are seeking a Registered Nurse (RN) to support part-time, interim basis. This role focuses on care coordination, clinical support, and interdisciplinary collaboration across multiple skilled nursing facilities. This job will have the following responsibilities:

We are seeking a Registered Nurse (RN) to support part-time, interim basis. This role focuses on care coordination, clinical support, and interdisciplinary collaboration across multiple skilled nursing facilities. This job will have the following responsibilities:
- Ensure accurate and complete clinical documentation
- Collaborate with interdisciplinary care teams and providers
- Support care coordination and patient management activities
- Assist with Medicare (Parts A, B, D) benefit determination
- Monitor and respond to changes in patient condition
- Coordinate therapy services and rehabilitation care
- Active RN license in CT (or ability to obtain by start date)
- Associate Degree or higher in Nursing
- Basic Life Support (BLS) Certification
- 1+ year of clinical experience (long-term care preferred)
- Experience managing patients with complex medical, behavioral, or social needs
