Responsibilities

• Perform comprehensive occupational therapy evaluations for SNF residents and develop individualized treatment plans to improve independence in activities of daily living and functional mobility.
• Implement evidence-based therapeutic interventions, monitor progress, and adjust goals and plans as needed; document outcomes and progress in the medical record.
• Collaborate with an interprofessional team to coordinate care, support discharge planning, and optimize resident outcomes across the continuum of care.
• Educate residents and families on home exercise programs, fall prevention, equipment use, and safety strategies to promote independence and safety.
• Ensure compliance with regulatory requirements and professional standards; participate in quality improvement activities and ongoing professional development.

Requirements

• Active OT license to practice in the United States (state-specific as required by law)
• Master’s or Doctoral degree in Occupational Therapy from an accredited program
• Ability to practice in a skilled nursing facility environment and to collaborate with an interdisciplinary team
• Strong communication and documentation skills; proficient in treatment plan development and progress notes
• Willingness to commit to a 13-week travel assignment and to work a five-day week
• BLS certification or equivalent recommended/required by the assignment (details provided during onboarding)
