Home-Visit Nurse Practitioner — Geriatrics

NONPF

UnitedHealthcare seeks a Nurse Practitioner to conduct clinical assessments of PH members, document encounters in EMR, and contribute to cost-effective, quality care. The NP reports to the NP Manager and will travel to member homes as required. Responsibilities include vitals, exams, diagnosing for care management, patient education, referrals, and HIPAA compliance.
