I help seniors living with chronic illnesses LIVE OUT LOUD and enjoy each day as much as possible, no matter what the doctor has written in their medical record. You know those seniors who think they may never walk again because they lost strength after being in the hospital too long? Yes, those seniors. We help them feel better, rebuild confidence and regain as much independence as possible.

The person I am looking for

I am looking for a newly licensed LPN who was a caregiver before becoming a nurse—someone who already understands that excellent care is personal. You notice the small things. You enjoy spending time with older adults. You can connect with families, coach caregivers and bring calm when care needs change.

You do not need to arrive as a finished clinical director. This is a paid leadership-development role. You will begin close to the client, learn our whole-person approach and personal care pathway, and grow under my mentorship as an experienced RN and family nurse practitioner.

What you will do

• Welcome and admit new clients through thoughtful intake and whole-person assessment.

• Serve as the first caregiver and clinical presence on a new case so you understand the client before building the team.

• Create the client’s personal care guide, including routines, risks, preferences, wellness goals and communication expectations.

• Help select, introduce and orient incoming Care Partners in the client’s home.

• Manage and coordinate care for moderate- to high-risk clients, with clear family communication and timely follow-through.

• Complete supervisory visits, medication setup and selected skilled nursing visits within the LPN scope and plan of care.

• Support wellness clinics and education inside retirement communities.

• Coach caregivers, participate in skills training and help improve care quality and consistency.

• Share limited clinical on-call coverage one to two weekends per month as the role develops.

• Grow into broader clinical leadership, care management and alternate-administrator responsibilities as readiness and regulatory requirements allow.

What we are looking for

• Current Illinois LPN license; newly licensed LPNs are welcome.

• Previous hands-on caregiver, CNA or direct-care experience before LPN licensure.

• A genuine interest in geriatric nursing, chronic illness management and aging in place.

• Warm, friendly and confident communication with clients, families and care teams.

• Reliability, sound judgment, initiative and willingness to learn.

• Ability to travel to client homes and partner communities across our service area.

• Experience in home care, home health, hospice, assisted living or senior living is strongly valued.
