Home Health LPN/LVNPride Health is hiring a Home Health LPN/LVN to support our client's healthcare facility in Spokane, WA.Job Details:Schedule: Monday-Friday | 8:00 AM-4:30 PMLocation: Spokane, WAJob Type: ContractContract Length: 13 WeeksPay Range: $34-$36/hr * Pay offered is based on experience, expertise, credentialing, and education.Duties:Provide high-quality clinical services within the scope of practice and in accordance with the plan of care.Continually observe and report patient nursing needs to the supervising RN.Communicate effectively with physicians, patients, family members, and agency staff regarding patient care needs, visit assignments, schedule changes, orders, OASIS data sets, and coding requests.Coordinate care with other clinicians and deliver services as ordered by the physician and in accordance with applicable laws and regulations.Participate in performance improvement initiatives to improve patient outcomes and reduce hospitalizations.Attend and actively participate in meetings, case conferences, and required training while maintaining compliance with agency standards.Participate in weekend rotation and potential holiday coverage as needed to meet patient care demands.Requirements:License(s): Active LPN/LVN LicenseYears of Experience: Home Health experience preferredSkills/Specialties/Technology: Patient care coordination, clinical documentation, OASIS knowledge, care planning, and strong communication skillsAbility to work independently in a home health setting and participate in an on-call/weekend rotation as needed.Equal Employment OpportunityPrideGlobal and it's affiliates are an equal opportunity employer.
